What's on this sheet
The mistake this sheet is really about is not the formula, it is the measurement you were handed. A child given the diameter who puts it where πr²h wants the radius is out by a factor of four, because the formula squares it. So each figure says which measurement it is showing, and the sheet can be narrowed to radius only, to diameter only, or left mixed — which is the hardest, since the child has to notice which they were given.
Why this sheet works
π is 3.14 here rather than 22/7. Every whole radius and height in range comes out exact at two decimal places, so no sheet needs a rounding instruction and no answer is a surd a thirteen-year-old cannot write in a blank.

8 ÷ 2 =4
base × height
base:3.14 × 4 × 4 =50.24
base:50.24
50.24 × 12 =602.88
- The 8 cm goes right across, so it is the diameter. Halve it first: the radius is 4. Every one of these formulas SQUARES the radius, so getting this wrong is out by a factor of four, not two.
- Same idea as a box: the area of the base times how tall it is. Here the base is a circle, and that is the only difference.
- The circle's area is π × r × r = 3.14 × 4 × 4 = 50.24 square cm.
- Times the height: 50.24 × 12 = 602.88 cubic cm.
